The Journey Transitioning Home provides safe, affordable housing and supportive services for adults transitioning from incarceration, homelessness, hospitals, rehabilitation, or other unstable situations.

Our mission
Our mission is to bridge the gap between crisis and stability. We provide housing, case management support, referral coordination, and community-based resources that help individuals regain independence and improve their quality of life.

Who we serve
We support adults ages 18 and older who are working toward stability and a fresh start, including people transitioning from jail or prison, experiencing homelessness, or leaving a hospital or rehabilitation stay.

Housing is the foundation. From there, we help people connect to the resources and support they need to move forward.

After release, people face barrier after barrier to the basics — work and a place to live. Without stable housing, a person is cut off from their community, far less likely to find or keep a job, and far more likely to end up back in incarceration.
