We serve adults ages 18 and older who are seeking stable independent living housing and supportive services while transitioning from incarceration, homelessness, hospitals, rehabilitation facilities, shelters, or other unstable living situations.
Yes. We can assist with resource navigation and document support, including help with applications for benefits and replacement documents such as identification cards, birth certificates, and Social Security cards.
We offer supportive services to help individuals work toward stability and independence. Services may vary based on individual needs, goals, and available community resources.
